Instructions for Consulting Services

Procedure for purchases of Academic or Research Consulting services
< $50,000:

Note: All purchases at $2,000 and above require a PO through Purchasing.

5 Items are Required:

1. Purchase Requisition
2. Proposal for Service (consultant must supply a statement of work to include a price quote to include all expenses, including travel, if applicable)
3. Cost/Price Analysis
4. Employee/Contractor Evaluation (use this form only for individuals-not companies)
5. W-9

Procedure for Consulting Services > $50,000:

If a particular consultant is the only one that can perform the services and competition is impracticable or unavailable, proceed as a Sole Source Purchase. A statement of work to include a written quote must accompany all sole source requests.

In addition, expenditures from Fund 2xxxxx over $2,000 require OSP approval. Consultant must complete W-9 form prior to issuance of a purchase order.
